  Hi there. I agree that it is very important. When an accident happens, the very last thing is that a human makes a mistake or a piece of equipment fails, but that’s only the end of a very long sequence of things happening. How did the person or equipment get into that situation? What could we do differently so that other people aren’t in the same situation?
